This module is designed to support interprofessional team members in their role development related to team-based care and care management.

In support of improving patient care, this activity has been planned and implemented by Beaumont Health and the Michigan Institute for Care Management and Transformation.  Beaumont Health is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.

Medicine CME: Beaumont Health designates this enduring material for a maximum of 1.0 AMA PRA Category 1 Credit(s)™.  Physicians should claim only the credit commensurate with the extent of their participation in the activity.

Nursing CE: Beaumont Health designates this activity for a maximum of 1.0 ANCC contact hour(s).  Nurses should claim only the credit commensurate with the extent of their participation in the activity.

Pharmacy CE: Beaumont Health designates this activity for 1.0 ACPE contact hour(s).
