  1. Describe key gaps in the quality of care for type 2 diabetes
  2. Practice combining CGM, medication and lifestyle interventions
  3. Appreciate quality improvement methodology as a tool to improve care
  4. Feel prepared to engage team members to participate in MCT2D initiatives

Instructions for Completion
