Objectives
  1. Describe key gaps in the quality of care for type 2 diabetes
  2. Practice combining CGM, medication and lifestyle interventions
  3. Appreciate quality improvement methodology as a tool to improve care
  4. Feel prepared to engage team members to participate in MCT2D initiatives

Registration

There is no fee for this module, however, registration is required.  If you don't already have a profile in the CE portal you will need to create one during the registration process.  
Instructions for Completion

1. Click Begin Activity
2. Click Register and complete the process. Click on the activity link at the bottom the confirmation pop-up window to return to the module (a direct link to the module is also provided in the registration confirmation email)
3. Click Launch Video
4. Upon completion of the module, click Attestation and answer the 1 attestation question, click Check Answer, click the green button in the upper right corner.                             

5. Complete the CE Evaluation (if you are not automatically directed to the evaluation upon completion of the attestation, go to My CME > Evaluations & Certificates)
